2022 US Stillbirth Scorecard

Stillbirth remains a pressing public health issue in the United States, affecting thousands of families each year. Yet, it often receives less attention than other pregnancy-related outcomes. The Stillbirth Scorecards provide a critical look at the statistics of stillbirth—both at the national level and within each state—helping us understand where we stand and where improvement is needed.

By examining these numbers, we can identify troubling trends and disparities in stillbirth rates. Some of these disparities are immediately visible in the data, such as the significantly higher stillbirth rates among Black and Indigenous families. Others, like the impact of socioeconomic status, access to quality healthcare, and environmental factors, may not be directly reflected in these numbers but are just as influential in shaping outcomes.

The Stillbirth Scorecards play a crucial role in raising awareness, which is essential for prevention efforts. Greater awareness leads to more conversations, better education, and stronger advocacy, all of which contribute to reducing the risk of stillbirth. Understanding the scope and scale of stillbirth, as well as the disparities that exist, allows us to advocate for better support, expand research, and implement targeted interventions to reduce preventable stillbirths.

Finally, these scorecards demonstrate the human impact of stillbirth. Each number represents a baby that is dearly missed by a family that will never be completely whole. We hope it will help families who know this pain to feel less alone. The data reinforces that they are not isolated in their experience and that stillbirth is a public health issue affecting many families. Shattering the silence around these deaths helps us honor their babies and work toward solutions while striving for a future where fewer families have to endure this heartbreak.

The Stillbirth Scorecard is designed to bring stillbirth out of the shadows and raise our collective awareness about the number of families being impacted by these deaths every day.  We must first understand the problem before we can fix it.

Scorecard Definitions

  • The rate looks at the number of stillbirths per 1,000 live births in the same time frame for the geography indicated.
  • The raw number indicates how many babies this represents in the year.
  • Disparity ratio is a calculation to look at how far apart the rate is in a racial/ethnic group with the worst stillbirth rate from the racial/ethnic group with the best stillbirth rate.  The goal for this calculation is 1.0 which would mean no difference in outcomes across racial/ethnic communities.  A larger number reflects more significant disparity in pregnancy outcomes.
  • The Cause of Death was added to this version of the scorecards.  They represent what is listed as an official cause of death on the fetal death reports, completed by the attending physicians and reported by each state to the CDC.  They can provide interesting information about what we know, and maybe more about what we don’t know about these tragic deaths.  It is important to note that these reports must be submitted within a few days of the baby’s death.  However, many test results are not available in this time.  Therefore, the diagnoses may be limited or inaccurate.  We feel this also tells a story about the data collection process.  Finally, there are many states with large numbers of ‘unknown’ or ‘unreported’ causes of death.  Our hope is that this can bring attention to the need for more and better testing as well as encouraging providers and government agencies to improve the testing and reporting systems.
  • The maternal age charts show the stillbirth rate for women at certain ages in that state.
  • The gestational age charts show the number of babies who died at the various gestational ages.
  • Autopsy Performed – percent of stillbirths evaluated with an autopsy per the fetal death report.
  • Stillbirth Tax Credit – indicates if the state offers a tax credit or deduction to families who experienced a stillbirth (as of 1/1/2023).
  • Certificate of Stillbirth – indicates if the state offers an official document (Certificate of Birth Resulting in Stillbirth or similar) to recognize stillborn babies.

Data Sources are identified on each state scorecard and include CDC Wonder Database and individual state offices of vital statistics.  CDC stillbirth data is only available through 2021.  The scorecards will be updated once additional data becomes available to the public
